APPLICATION TO PASADENA CHRISTIAN PRESCHOOL:

ENROLLMENT:
All new students are required to submit the following forms, required by the State of California’s Community Care Licensing (Hard copies available at the preschool):

IMMUNIZATIONS:
All students must be current on immunizations OR have a physician’s exemption to attend school. Please CLICK HERE to see which shots are required for child care/preschool entry.
BIRTH CERTIFICATE:
A birth certificate or other form of government identification, such as a passport, is required for proof of age prior to entry.
MEDICATION:
By law, we cannot administer any non-prescription medication, lotion, or cream unless authorized by a parent. This includes sunscreen and first aid ointments. Medication must be in the original sealed package.

Prescription Medication must be authorized by your child’s physician. Prescriptions must be brought in their original prescription bottle with the physician’s instructions on the label.

ALLERGIES:
We take the health and well-being of our students very seriously, so it is our policy that each child has an Allergy Action Plan on file, regardless of severity.
